What information does the out-of-hours GP service need to know about me?

The triage assistant asks for the following patient information, among others:
If you are calling for someone else, you will also need the following information for them.

• Your date of birth, name, postcode and house number
• Your phone number
• The name of your GP
• Your health insurance number
• Description of your complaint(s)
• Your medical history (major condition/diseases/allergies)
• Your medication use
• Proof of identity (are you going to the out-of-hours GP service? Please bring your ID)
